Ten years ago, on the 9th of June 2016, the Government of India launched the Pradhan Mantri Surakshit Matritva Abhiyaan, or PMSMA, with a simple but powerful objective – to ensure that every pregnant woman in the country has access to quality antenatal care, timely screening, and specialist medical attention during pregnancy. The initiative was designed to address one of India’s most pressing public health challenges: maternal mortality. At the time, India’s Maternal Mortality Ratio stood at 130 maternal deaths per lakh live births. Early identification of high-risk pregnancies, regular antenatal check-ups, specialist consultations, and timely referrals were identified as critical interventions to save lives and improve maternal and newborn health outcomes. A decade later, the scale and impact of PMSMA are evident. More than 7.5 crore pregnant women have received antenatal care services under the programme. Millions of high-risk pregnancies have been identified and monitored, ensuring that women who need specialised care receive it at the right time. The programme has expanded from a monthly antenatal care initiative to a comprehensive system of tracking and follow-up through the Extended PMSMA strategy launched in 2022. The results are reflected in India’s maternal health indicators. The country’s Maternal Mortality Ratio declined from 130 to 87 per lakh live births between 2014–16 and 2022–24. But beyond the numbers lies a larger story – of strengthening healthcare systems, empowering the frontline health workers, improving access in remote and underserved regions, and making maternal healthcare more inclusive, accessible and responsive. So, the transformation in maternal healthcare delivery in India, what have been the key drivers of its success, and what challenges remain, particularly in reaching the last mile.
